A rare differential diagnosis of subglottic stenosis

    Introduction:

    Primary involvement of the larynx in non-Hodgkin's lymphoma (NHL) is rare. They represent < 1% of all laryngeal tumors. Usually laryngeal NHL occurs in the supraglottic area sometimes compared with infiltration of the glottic part. Primary involvement of the subglottic region is rarely reported.

    Case-report:

    A 29-year-old man was presenting with exertional dyspnea, exertional stridor and dysphonia in our hospital. He suffered from these symptoms existed since two months without B symptoms. He got 250 mg prednisolone intravenously in case of suspected subglottic stenosis in another hospital, but symptoms didn't change. A CT scan of the neck and thorax revealed a small hyperplasia of the soft tissue in the subglottic region (right > left). In the clinical examination vocal cords appeared grey with a smooth surface, freely movable with suspicious additional subglottic tissue. For further diagnostics tracheobronchoscopy and laryngoscopy were performed under general anesthesia. On both sides of the subglottic area suspicious mucosa appeared. A biopsy was taken.

    Results:

    The histological examination verified a lymphoid cell population with active proliferation. Subglottic manifestation of B-cell lymphoma or acute lymphoblastic leukemia seems to be possible.

    Conclusion:

    This case-presentation should to draw attention to rare differential diagnosis of subglottic stenosis. Beside ENT- and internistic diseases we should also think of haematological reasons.
