Estimation of prosthesis lengths using digital surgery microscopes - experiments in human temporal bones

    Introduction Digital surgery microscopes with 3D imaging and in situ distance measurement open up new perspectives in middle ear surgery. Intraoperative determination of prosthesis lengths for ossicular reconstruction is of particular interest.

    Methods Measurements were performed in 7 human temporal bones. After tympanotomy, several points were registrated with ARRIscope. With the help of these points, the prosthesis’ shape and length can generally be determined. Using a TORP, two different visual angles are needed to define all the necessary points. After registration of identical reference points from both angles, coordinates could be transformed and evaluated altogether.

    Results A common coordinate system was calculated by coordinate transformation, offering various further calculations. Deviations of transformed coordinates were under 0.1 mm, a high accuracy can be assumed.

    Conclusion An in situ measurement of prosthesis lengths and shapes during ossicular reconstruction seems possible with reasonable accuracy.
