EUS-Guided Percutaneous Endoscopic Jejunostomy In Bariatric Surgery (Single Anastomosis Duodeno-Ileal Switch- Sadis)
51-year-old male with bulbar ALS, bariatric surgery (SADIS-S type) reconverted to Roux-en-Y bypass. Absence of transillumination prevents from conventional PEG placement. Proposal for EUS-guided percutaneous feeding tube placement.
Percutaneous EUS-target creation with a water-filled glove placing it over the abdomen. Initial attempt to identify EUS-target from stomach, not possible for excessive distance. Advancement to jejunum, EUS-guided transjejunal puncture and advancement of a guidewire, knotting it to feeding system and passed into the bowel and drawn back through the mouth. Finally, PEJ placement following traditional technique.
