Facultative myiasis in pT4 squamous cell carcinoma of the left auricle

    Introduction: A 95-year-old patient presented to the emergency department with myiasis of an ulcerated tumor with disintegration of the left auricle. The patient was admitted and local therapy of the ear with removal of the individual maggots(approx. 30) with tweezers was performed. The ulcerating auricle was cleaned with an antiseptic (Schülke Octenisept®) and covered with iodoform strips, compresses and sterile bandages. Additionally, an intravenous antibiotic therapy with ampicillin / sulbactamwas administered. After taking the sample in LA, the tissue examination showed evidence of a squamous cell carcinoma. Due to the advanced age and underlying cardiological conditions, the patient did not qualify for therapeutic measures such as surgery and radiationtherapy. Local improvement was seen as a result of the treatment measures. Three months later, the patient presented again with a recurrent myiasis. The same therapeutic measures were taken. A few hours after admission to hospital, the patient died of cardiacfailure. Conclusion: Myiasis continues to be a rare disease in Germany, which mainly occurs in elderly patients with chronic skin lesions. It is recommended that these patients cover their wounds to protect them from colonization by insects. Furthermore,those in need of care should be treated according to established hygiene standards, in order to prevent insect infestation.
