Comparison of treatment options for obstructive sleep apnea based on mean disease alleviation
Introduction Various options are available for the therapy of OSA. A direct comparison is difficult because they depend to varying degrees on patient compliance. Mean Disease Alleviation (MDA) includes not only effectiveness but also compliance. The aim is to identify the MDA of the most important therapy options for OSA or to calculate it using available data.
Methods The MDA is defined as the product of therapeutic effectiveness and adjusted compliance / 100. The adjusted compliance is calculated from the use of the therapy depending on the length of sleep. The therapeutic effectiveness is the reduction of the initial AHI under therapy given as a percentage. A literature search was carried out on “mean disease alleviation” and “sleep apnea”. Meta-analyzes were used to calculate the MDA of treatment options for which no information was identified.
Results Publications were identified that calculated the MDA of PAP therapy, therapy with MAD and positional therapy. It was possible to calculate the MDA for TE / UPPP and maxillomandibular advancement (MMA), since the adjusted compliance here is 100%. The MDA for PAP therapy was 52.4%, for MAD it was 51.1% and for positional therapy it was 42%. An MDA of 49.5% could be calculated for the TE / UPPP and an MDA of 83.4% for the MMA. The stimulation therapy of the N. XII is a special feature, since no data has yet been published that would allow the calculation of the adjusted compliance.
Conclusion The calculation of the MDA enables a comparison between different forms of therapy taking into account the actual usage time.
