Curative ESD of a large rectal tubulovillous adenoma with high grade dysplasia

    Abstract Text A 42-year-old female presented with rectal bleeding. Colonoscopy revealed a large rectal polyp measuring 6cm located 2cm above the anal verge. After marking the borders, submucosal injection was followed by a circumferential mucosal incision. Due to extensive fibrosis, traction was applied at multiple points using the clip/string technique to facilitate submucosal dissection. Final pathology showed a tubulovillous adenoma with focal high-grade dysplasia. Follow-up 9 months later showed a clean scar without residual adenoma.
