Combined Endovascular Embolization and Far Lateral Craniectomy for Microsurgical Trapping and Resection of Giant Thrombosed Vertebro-Basilar Aneurysm

    We describe the case of a 29-year-old male who presented with a giant, calcified, partially thrombosed vertebrobasilar aneurysm with significant brainstem compression. Endovascular embolization of the aneurysm's mural channel was performed for distal control, followed by a far lateral, retrosigmoid, and suboccipital craniectomy and C1 laminectomy for exposure and transection of bilateral vertebral arteries distal to PICA. Resection of the thrombosed portion of the aneurysm was performed with an ultrasonic aspirator to achieve brainstem decompression.
