Behandlungsziele im Disease–Management–Programm erreicht? – DMP–Programme als Netzwerk

Seit 2003 sind „Disease–Management–Programme” (DMP) zur Verbesserung der Behandlung von chronisch kranken Patienten in Deutschland eingeführt. Ziel der Behandlung ist die Vermeidung bzw. das Hinauszögern von Komplikationen durch eine strukturierte Behandlung und kontinuierliche Überwachung der Patienten. Ein Kernelement der DMP–Programme ist das Qualitätsmanagement und die wissenschaftliche Begleitung aufgrund der erhobenen Daten. Ein Modell hierfür ist die Gemeinsame Einrichtung von Krankenkassen und Ärzten in Nordrhein. Anhand des DMP–Programms „Diabetes mellitus Typ 2”, in dem ca. 70  % der Patienten in Nordrhein erfasst sind, werden das Qualitätsmanagement und die wissenschaftliche Begleitung sowie die bisher erreichten Ziele beschrieben. Dabei wird gezeigt, dass die meisten Qualitätsziele erreicht werden.

„Disease Management Programs” (DMP) have been introduced in Germany since 2003 to improve the treatment of chronically diseased patients. Target of these programs is to prevent or at least to delay complications by means of structurized treatment and continual control of the patients. One of the main features of these DMP programs is quality control and scientific follow–up based on the observed data. A model set–up is the co–operative team of statutory sickness insurance bodies and physicians that has been established in North–Rhine Westphalia, one of the governmental administrative regions in the Federal Republic of Germany. Basing on the DMP–program „Diabetes mellitus Type 2” comprising about 70  % of the patients in North Rhine Westphalia, the quality management and scientific follow–up as well as the achieved targets are described below. It is shown that most of the aims are realized.
