Zusammenfassung

Auch schwere rheumatische Veränderungen des Glenohumeralgelenkes und seiner periartikulären Weichgewebe lassen sich erfolgreich durch die Schulterendoprothetik behandeln. Das Spektrum der chirurgischen Optionen ist breit. Die Endoprothetik bietet verschiedene Prothesentypen mit jeweils spezifischen Vor- und Nachteilen. Die rheumatische Arthritis (RA) und andere entzündlichrheumatische Krankheiten weisen gravierende Unterschiede zu den Arthrosen auf, das betrifft die Indikationsstellung, die Operationsvorbereitung, die Durchführung der Operation und die Nachbehandlung. Die Indikationsstellung und die Operation sollten in der Hand eines erfahrenen Rheumachirurgen liegen, der die Spezifika der entzündlich rheumatischen Erkrankungen in Abgrenzung zur Arthrose dezidiert erfahren hat und in Kenntnis des gesamten Spektrums der Differenzialindikationen und Prothesenmodelle eine individuelle Entscheidung für und mit dem Patienten trifft.

Summary

Even in severely destroyed rheumatoid shoulders endoprosthetic replacement offers successful options for treatment. In principle the rheumatoid shoulder differs significantly from the osteoarthritic shoulder, as well regarding preoperative soft tissue and bone conditions as regarding tissue healing. These differences strictly must be taken into account when indicating endoprosthetic replacement of the shoulder in the rheumatoid patient. The specific condition of the inflammatory disease must be considered during the surgical procedure and the rehabilitation program. There is a wide range of surgical options and a large amount of different types of endoprostheses with specific profits and concerns. The surgeon should be well experienced with rheumatoid arthritis surgery. The individual needs of the patient must be regarded for surgery of the rheumatoid shoulder.
