Zusammenfassung

Die Schultergelenksbeteiligung beim juvenilen Rheumatiker manifestiert sich klinisch wie auch radiologisch erst sehr spät. Deswegen ist eine frühzeitige Detektion über die Sonografie oder ein MRT wegweisend. Ein Gelenkerhalt ist unter einer suffizienten medikamentösen Therapie über die Synovialektomie dauerhaft möglich, in späteren Stadien kann nach Destruktion die Hemiprothese oder bei Rotatorenmanschettendefekt die inverse Schulterendoprothetik ein therapeutisches Instrument sein, Schmerz unter Funktionsgewinn zu reduzieren.

Summary

Destruction of the shoulder joint juvenile rheumatism causes less pain and functional deficiency over a long period, radiological signs appear very late. Therefore, early detection of synovitis by ultrasonography or MRI is essential, for early diagnosis and joint preserving therapy. Joint preservation is possible by a sufficient immunosuppressive drug therapy combined with a surgical synovectomy. In later stages of destruction (Larson IV and V) hemiarthroplasty or, in the case of cuff tear arthropathy, inverse shoulder endoprosthesis are therapeutic options to reduce pain and improve function.
