Das Implantat im kieferorthopädischen Kontext – die Agenesie in der Frontzahnregion

Abstract

Agenesien von bleibenden Zähnen werden mit einer Prävalenz von bis zu 8% der Bevölkerung angegeben und Agenesien der lateralen Schneidzähne sind am dritthäufigsten betroffen. Die wichtige Frage für den behandelnden Kieferorthopäden ist, ob ein Lückenschluss oder eine Lückenöffnung als Therapiekonzept gewählt wird. Die Tendenz in der Literatur geht ganz deutlich Richtung Lückenschluss, was bei Vorliegen einer Klasse II-Relation oder bei Platzmangel als typische Option gewählt wird. Jedoch entsteht der Eindruck, dass auch massive Zahnbewegungen mit Gefahr von Wurzelresorptionen in Kauf genommen werden, um den Lückenschluss als Vermeidung von prothetischer Versorgung umzusetzen. Aus historischer Sicht betrachtet waren die Erfolgsraten und die ästhetischen Ergebnisse der Prothetik nicht immer zufriedenstellend. Der vorliegende Artikel soll die modernen und aktuellen Behandlungsoptionen zur Lückenversorgung vom navigierten Implantat bis zur einflügeligen Zirkonoxid-Klebebrücke darstellen. Das Thema der Milchzahnpersistenz im Frontzahngebiet mit wird ebenfalls beleuchtet.

Abstract

Permanent tooth agenesis is reported to have a prevalence of up to 8% of the population and lateral incisor agenesis is the third most commonly affected. The important question for the treating orthodontist is whether space closure or space opening is chosen as the therapy concept. The trend in the literature is clearly towards space closure, which is the typical option chosen when there is a class II relationship or when there is a lack of space. However, the impression arises that massive tooth movements with the risk of root resorption are accepted in order to close the space to avoid prosthetic reconstruction. Historically, the success rates and esthetic outcomes of prosthetics have not always been satisfactory. This article is intended to present the modern and up-to-date treatment options for the restoration, from navigated implants to single-retainer zirconia-ceramic resin-bonded adhesive bridges. The topic of milk tooth persistence in the anterior region is also highlighted.
