Zusammenfassung
Die Integrität der Thoraxwand und damit der Schutz der thorakalen Organe ist essenziell für das Überleben. Diese Integrität ist jedoch durch eine Vielzahl von Faktoren gefährdet, seien es Tumoren, Wundheilungsstörungen oder Verletzungen. Eine adäquate chirurgische Therapie dieser Erkrankungen erfordert häufig (Teil-)Resektionen der Thoraxwand, welche mit einer entsprechenden Kompromittierung der Barrierefunktion selbiger einhergeht. Eine sichere, schnelle und möglichst schonende Rekonstruktion von Defekten der Thoraxwand ist daher anzustreben und kann durch die Nutzung rekonstruktiver Operationsverfahren erreicht werden. Ziel dieses Übersichtsartikels ist es daher, die Möglichkeiten der Defektdeckung im Bereich der Thoraxwand an Beispielen aus dem eigenen Patientenklientel darzustellen und sie im Kontext der Literatur zu gewichten, um so einen Algorithmus für das rekonstruktive Vorgehen aufzuzeigen.
Abstract
The integrity of the thoracic wall and therefore the protection of the thoracic organs is paramount for survival. This integrity can be compromised by various factors, be it tumours, wound-healing disorders or injuries. An adequate surgical therapy for these entities often requires (partial) resections of the thoracic wall resulting in a loss of its protective function. A safe and timely reconstruction of the thoracic wall is therefore mandatory and can be achieved by means of reconstructive procedures. This article therefore aims to illustrate such reconstructive procedures utilising our own clinical cases while also giving a review of the literature in order to construct an algorithm for the reconstruction procedure.
