Summary
The case of a patient is presented who was frequently awakened during wake-to-sleep transition by an exploding feeling in his head. This is called exploding-head-syndrome and is a harmless but not well known phenomenon. Nothing is known about its pathophysiology. Mainly, elderly people are affected during wake-to-sleep transition; it is often accompanied by a frightening feeling, but only rarely by headache. Since patients often report this phenomenon in headache clinics, it should be known to headache specialists.
