Penetration eines spannungsfreien Vaginalbandes (TVT) in die Urethra - Ein Fallbericht

Zusammenfassung

Wir berichten über eine urethrale Penetration einer Polypropylenschlinge nach TVT-Operation. Die Patientin stellte sich fünf Monate nach einer unkompliziert durchgeführten Operation vor. Es bestand eine Dysurie und ein verlangsamter Miktionsablauf. Die Urethrozystoskopie zeigte eine Perforation des Bandes in die Urethra. Es wurde eine offene Urethrotomie bei 6 Uhr mit einer Teilentfernung des Bandes und einer Harnröhrenrekonstruktion durchgeführt. Nach dieser Maßnahme war die Patientin erneut inkontinent. Deshalb folgte 10 Wochen später eine abdominale Hysterektomie mit Sakrokolpopexie und Kolposuspension nach Cowan, worauf die Patientin kontinent war.

Abstract

We report on a case with urethral erosion of a polypropylene sling after TVT-procedure. The patient presented to our hospital five months after an uneventful operation. She complained of dysuria and an increase in the voiding time. The urethrocystoscopy showed migration of the tape into the urethra. Therefore we performed an open urethrotomy at the 6 o'clock position with resection of a part of the tape and urethral reconstruction. After that procedure the patient developed a recurrence of urinary incontinence. Therefore, 10 weeks later, we performed a hysterectomy with a sacrocolpopexia and a colposuspension according to Cowan. After that procedure the patient was continent.
