Zusammenfassung
Im Rahmen einer konsekutiven Fallkontrollstudie an den Univ.-HNO-Kliniken Heidelberg und Gießen wurde der Stellenwert des chronischen Tabak- und Alkoholkonsums für die Entstehung von Plattenepithelkarzinomen im oberen Aerodigestivtrakt analysiert. Die Untersuchung erfolgte an 200 Patienten mit Plattenepithelkarzinomen im Bereich von Mundhöhle, Oropharynx, Hypopharynx und Larynx und an 800 Kontrollpersonen (4:1 gematchtes Design). Lediglich 4,5% der Tumorpatienten gaben an niemals geraucht zu haben, im Gegensatz zu 29,5% bei den Kontrollpersonen. Der mittlere Tabakkonsum im Tumorkollektiv war ebenso wie der mittlere Alkoholkonsum mehr als doppelt so hoch wie im Kontrollkollektiv, wobei die Patienten mit Oropharynxkarzinomen mit Abstand am meisten rauchten und tranken. Das tabakassoziierte (alkoholassoziierte) relative Risiko (RR) an einem Plattenepithelkarzinom im Bereich des oberen Aerodigestivtraktes zu erkranken, ließ nach statistischer Bereinigung möglicher Alkoholeffekte (möglicher Tabakeffekte) eine signifikante Dosis-Wirkungs-Beziehung erkennen: Bei einem Konsum von mehr als 60 Packungsjahren (PJ) kam es zu einem Anstieg des RR auf das 23,4fache des Ausgangswertes; bei einem Konsum von mehr als 100 g Alkohol/Tag kam es zu einem Anstieg des RR auf das 21,4fache des Ausgangswertes. Eine vorläufige Analyse des tabakassozierten (alkoholassoziierten) Krebsrisikos für unterschiedliche Tumorlokalisation ergab für Mundhöhlen-und Larynxkarzinome (Oropharynx- und Larynxkarzinome) die höchsten Risikowerte. - Kombinierter Tabak- und Alkoholkonsum zeigten hinsichtlich des RR an einem Plattenepthelkarzinom des oberen Aerodigestivtraktes zu erkranken eine synergistische Wirkung: Bei einem Tabakkonsum von über 50 PJ und einem Konsum von mehr als 75 g Alkohol/Tag stieg das Risiko auf das 146,2fache des Ausgangswertes an.
Summary
A case-control study of squamous cell carcinoma of the upper aerodigestive tract conducted in the ENT Departments of the University hospitals of Heidelberg and Giessen (FRG) provided information on the role of chronic tobacco and alcohol consumption as risk factors in 200 patients and 800 control subjects (4:1 matched design). - Only 4.5% of the tumour patients were non-smokers in contrast to 29.5% in the control group. The average tobacco and alcohol consumption in the patients was more than twice that high than in the control subjects. The highest alcohol and tobacco consumption was observed in patients suffering from oropharyngeal cancer. - It could be demonstrated that tobacco as well as alcohol increased the risk for head and neck cancer, doese-dependently, as separate risk factors. In heavy smokers (> 60 pack years) a relative risk of 23.4 (alcohol-adjusted) was calculated. For heavy drinkers (> 100 g ethanol/day) a relative risk of 21.4 (tobacco-adjusted) was documented. Combined alcohol and tobacco consumption showed a synergistic effect and increased the cancer risk more in an multiplicative than in an additive manner: A daily ethanol consumption of more than 75 g/day combined with more than 50 pack years caused a risk value of 146.2.
