Abstract

The transseptal frontal sinusotomy (TSFS) utilizes the unique relationship of the nasal septum to the medial frontal sinus floor and allows for entry into the midline frontal sinus floor where the bone is thinnest. This technique is especially advantageous in the cases of extensive neo-osteogenesis that preclude identification and cannulation of the frontal recess. Similar to the Draf III and the endoscopic modified Lothrop procedure, TSFS provides good access to the midline frontal sinus floor and permits but does not require intersinus septum removal. 1 , 2 In addition, TSFS offers several theoretical advantages over frontal sinus obliteration including decreased morbidity, improved cosmesis, and ease of endoscopic and radiographic surveillance postoperatively. A concise review of the indications, procedural steps, and complications will be presented. An illustrative example with be shown and potential points of difficulty during the procedure will be addressed.
