Akute psychotische Störungsbilder

Akute psychotische Störungsbilder sind nicht auf organische Ursachen oder Substanzeinfluss zurückzuführen. Sie zeichnen sich durch einen akuten Beginn sowie diverse Symptome aus, die sowohl affektiver als auch produktiv-psychotischer und psychomotorischer Art sein können. Die Beschwerden remittieren meist binnen kurzer Zeit vollständig ohne nennenswerte Residuen, neigen jedoch zu rezidivierenden Verläufen. Auch sind Übergänge in andere Krankheitsbilder möglich.

Akute psychotische Störungsbilder (APSt) sind gekennzeichnet durch einen akuten Krankheitsbeginn und das Nebeneinander psychotischer, affektiver und psychomotorischer Symptome, die innerhalb kurzer Zeit wieder remittieren.

Der Nachweis einer definierbaren somatischen Ursache sowie Drogenkonsum schließt die Diagnose aus.

Die Prognose ist prinzipiell günstig, jedoch ist die Rezidivrate hoch.

Wichtigste Differenzialdiagnose ist das Delir, weswegen eine umfassende Abklärung erforderlich ist.

Frauen erkranken häufiger als Männer; Männer erkranken jedoch früher als Frauen und weisen häufiger schizophrenietypische Symptome auf.

Weibliches Geschlecht, spätere Erstmanifestation und hohe Akuität des Symptombeginns gelten als prognostisch günstige Faktoren.

Akute Krankheitsphasen gehen zuweilen mit einer erheblichen Eigen- und/oder Fremdgefährdung und einer erhöhten Mortalität einher. Die wichtigste Akutmaßnahme besteht daher im Schutz des Patienten und seiner Umwelt.

Ein früher Behandlungsbeginn verbessert die Prognose, weswegen Benzodiazepine und Antipsychotika früh zum Einsatz kommen sollten. Unklar ist jedoch, wie lange die Medikation fortgeführt werden sollte.

Im langfristigen Verlauf besitzen APSt eine nur geringe Diagnosestabilität, und Übergänge in andere Erkrankungen sind häufig.
