Commentary

Self-expanding metal stents (SEMSs) are useful to palliate malignant and benign gastrointestinal strictures and to bridge the gastrointestinal lumen in cases of leaks and fistulas. Occasionally, the stent axis may not align itself with the luminal gastrointestinal axis resulting in additional problems such as recurrent obstruction. As shown in this E-Video by Mangiavilliano et al., this problem can be solved by clipping on the proximal part of the SEMS located on the lesser curvature of the stomach. The authors clearly show that by approximating the mesh with the clip, the length of the SEMS is shortened in this location appropriately curving it and adapting the SEMS to the axis of the stomach lumen.
