Vergleichbarkeit subjektiver, durch den Patienten beantworteter Feedbackfragebögen und klinischer Fragebögen zur Beurteilung der Schulterfunktion bei Patienten mit proximaler Humerusfraktur

Zusammenfassung

Hintergrund Im Rahmen des Qualitätsmanagements nehmen vom Patienten subjektiv beantwortete Feedbackfragebögen (PROMs) an Bedeutung zu. Verschiedene PROMs und klinische Fragebögen stehen dem Untersucher zur Beurteilung der Schulterfunktion nach proximaler Humerusfraktur (PHFx) zur Verfügung. In Europa gehören neben dem Constant-Murley-Score (CS) der Neer-Score (NS), der Oxford-Shoulder-Score (OSS), der University of California at Los Angeles Score (UCLA) und der Disabilities of the Arm, Shoulder and Hands Score (DASH) dazu. Darüber hinaus kann die gesundheitsbezogene Lebensqualität durch die PROMs Short Form 36 (SF-36) und EuroQol (EQ-5D) eingeschätzt werden. Obwohl all diese Testinstrumente die Schulterfunktion beurteilen, variieren die objektiv und subjektiv zu beantwortenden Komponenten und somit die Möglichkeit zur eigenständigen Beantwortung. Ziel der vorliegenden Studie ist es, die Übereinstimmung der Ergebnisse der unterschiedlichen PROMs und der klinischen Fragebögen miteinander zu vergleichen.

Methode In die Studie wurden 76 Patienten eingeschlossen, die zwischen 01/2001 und 12/2005 bei einer proximalen Humerusfraktur mit einer winkelstabilen Plattenosteosynthese versorgt wurden. Das Ergebnis wurde mit den PROMs bzw. klinischen Fragebögen CS, NS, OSS, UCLA, DASH, SF-36 und EQ-5D bewertet und ein Korrelationskoeffizient errechnet. Zusätzlich wurde beim CS zwischen den 2 Kraftmessmethoden (Handgelenk [HG] vs. Deltamuskel [DM]) unterschieden.

Ergebnisse Die Korrelation der Ergebnisse von CS und NS (HG: r = 0,85; p < 0,001/DM: r = 0,93; p < 0,001), CS und UCLA (HG: r = 0,83; p < 0,001/DM: r = 0,86; p < 0,001), NS und UCLA (r = 0,91; p < 0,001) sowie DASH und OSS (r = 0,88; p < 0,001) zeigte sich stark ausgeprägt. Eine gute Vergleichbarkeit der Ergebnisse konnten wir zwischen CS und OSS (HG: r = 0,63; p < 0,001/DM: r = 0,66; p < 0,001) und zwischen CS und DASH (HG: r = 0,62; p < 0,001/DM: r = 0,61; p < 0,001) nachweisen. Ebenfalls gut zeigte sich die Korrelation des CS (HG/DM) und UCLA mit dem EQ-5D-Index. Die Beurteilung der körperlichen Leistungsfähigkeit des SF-36 korrelierte moderat bis stark mit dem CS, NS, OSS, UCLA und DASH, während die mentalen Komponenten des SF-36 eine schwache Korrelation zeigten (p > 0,05).

Schlussfolgerung Die Beurteilung der Schulterfunktion nach proximaler Humerusfraktur zeigte eine sehr starke Korrelation innerhalb der klinischen Fragebögen (CS/NS/UCLA) bzw. der PROMs (OSS/DASH). Eine starke Korrelation bestand zudem zwischen den klinischen Fragebögen und den PROMs. Die Korrelation mit dem EQ-5D zeigte sich moderat. Die moderate bis starke Korrelation zwischen den physischen Komponenten, bei nahezu unabhängiger Korrelation zwischen den mentalen Komponenten des SF-36, zeigt, dass die Einschränkung der Lebensqualität in unserer Kohorte auf einer physischen, nicht jedoch auf einer psychischen Beeinträchtigung beruht.
