Subdural hemorrhages in children
ZUSAMMENFASSUNG
Subdurale Hämatome entstehen im Kindesalter überwiegend traumatisch. Neben Verkehrsunfällen, Stürzen und geburtstraumatischen Verletzungen stellt im Säuglingsalter das Schütteltrauma eine der häufigsten Ursachen der akuten Subduralblutung dar. Pathophysiologisch liegt eine venöse Einblutung zwischen Arachnoidea und Dura mater aus den Brückenvenen vor. Weniger die Subduralblutung selbst als die begleitenden zerebralen Verletzungen können rasch zu einem lebensbedrohlichen Krankheitsbild führen. Ausführliche Anamnese und Untersuchung hinsichtlich des Vorliegens weiterer Verletzungen können Hinweise auf ein nicht-akzidentelles Entstehen geben.
ABSTRACT
Trauma is a major cause of subdural hemorrhages in children. Abusive head trauma is the most common cause of acute subdural bleeding in infants apart from traffic accidents, falls and birth traumas. The pathophysiological etiology of subdural hematoma is a venous bleeding between archnoidea and dura mater from the bridging veins. Concomitant intracerebral injuries may cause abrupt life-threatining situations. Detailed medical and social history and examination for additional injuries are possible indicators for abusive head trauma.
