Global Child Health – Kindergesundheit auf der ganzen Welt

Immer noch sterben weltweit zu viele Kinder aufgrund mangelnden Zugangs zu Prävention und angemessener Behandlung. Qualitätsstandards und die Ausbildung der Gesundheitsarbeiter werden durch Programme wie das IMCI verbessert und durch Richtlinien wie die WHO Pocket Books „Hospital Care for Children“ und „Primary Health Care for Children and Adolescents“ gesetzt [1] [2]. Sie sind die Basis für globale Strategien und Qualitätsverbesserungsprogramme.

Kernaussagen

• Die globale Kindersterblichkeit hat sich signifikant verringert, dennoch sterben immer noch zu viele Kinder an vermeidbaren oder behandelbaren Erkrankungen.

• Haupttodesursachen sind, neben der Säuglingssterblichkeit, Durchfall und Pneumonie bei Kindern < 5 Jahren. Bei älteren Kindern und Jugendlichen sind neben Infektionen (Verkehrs-)Unfälle, Gewalt, Suizide, Schwangerschaftskomplikationen und nicht übertragbare Erkrankungen die Haupttodesursachen.

• Die extremen Ungleichheiten zwischen armen und reichen Ländern sowie der reichen und armen Bevölkerung innerhalb eines Landes bestehen weiterhin.

• Die Qualität der Versorgung von Kindern ist immer noch unzureichend und ein wichtiger Grund für die Kindermortalität und Morbidität.

• Evidenzbasierte internationale Richtlinien, die national adaptiert und umgesetzt werden, können die Situation verbessern.

• Gute Versorgung bei der Geburt, Prävention und Behandlung von Atemwegs- und intestinalen Infektionen, Malaria, HIV/TB und Mangelernährung könnten einen Großteil der Sterblichkeit verhindern.

• Nicht übertragbare Krankheiten spielen auch im Kindes- und Jugendalter eine zunehmende Rolle.

• In vielen Ländern ist der Gesundheitssektor unterfinanziert und die Bezahlung und Ausbildung des Gesundheitspersonals unzureichend.
