ZUSAMMENFASSUNG
Die axiale Spondyloarthritis (axSpA) ist eine häufige entzündlich rheumatologische Erkrankung, die durch axiale Symptome sowie periphere und extramuskuloskelettale Manifestationen gekennzeichnet ist. Die axSpA umfasst 2 Stadien, die nicht-röntgenologische (nr-) und die röntgenologische (r-) axSpA, auch als ankylosierende Spondylitis bezeichnet, die sich anhand definierter röntgenologischer Kriterien unterscheiden und als Krankheitskontinuum verstanden werden. Patienten mit nr-axSpA stellen frühe Formen der axSpA dar, bei dem einige, aber nicht alle Patienten, schließlich zur r-axSpA fortschreiten. Die Einteilung in diese Stadien wird vor allem für klinische Studien angewendet. Obwohl sich die zeitliche Verzögerung bis zur Diagnosestellung bei axSpA in den letzten Jahren gebessert hat, ist sie im Mittel mit 5–6 Jahren jedoch weiterhin recht lang. Da die Erkrankung auch hinsichtlich struktureller knöcherner Veränderungen im Achsenskelett einen progredienten Verlauf nehmen kann, kommt der frühen Diagnosestellung und der damit verbundenen frühen Therapie eine wichtige Rolle zu. Gezielte Überweisungsstrategien und Früh-/Screeningsprechstunden können helfen Patienten schneller rheumatologisch vorzustellen, zu diagnostizieren und effektiv zu behandeln.
SUMMARY
Axial spondyloarthritis (axSpA) is a chronic inflammatory rheumatic disease and is characterized by axial symptoms as well as peripheral and extra-musculoskeletal manifestations. AxSpA comprises 2 stages, non-radiographic (nr-) and radiographic (r-) axSpA, also referred to as ankylosing spondylitis which are distinguished on the basis of defined radiographic criteria and are understood as a disease continuum. Nr-axSpA is considered as early axSpA in which some, but not all, patients eventually progress to r-axSpA. The division in these 2 stages is mainly used for clinical trials. Although the time delay to diagnosis in axSpA has improved in recent years, it remains quite long on average at 5–6 years. Since the disease may have a progressive course in terms of structural changes in the axial skeleton, early diagnosis and early therapy play an important role. In order to reduce the prognostically relevant time interval between the initial manifestation diagnosis as well as the consecutive initiation of an appropriate treatment, models of early recognition/screening consultations help to improve early detection of axSpA patients.
