Einfache Leberzysten

Einfache Leberzysten sind runde oder ovale, flüssigkeitsgefüllte Strukturen im Leberparenchym, die auf Basis einer Fehlbildung der Gallengänge entstehen. Die überwiegenden Anteile der Zysten sind klinisch stumm und werden als Zufallsbefund entdeckt. Auffällige Zysten verursachen eher unspezifische Symptome durch ihre Größe, ihr Wachstum und raumfordernden Charakter. Während bei stummen Zysten ein abwartendes Verhalten gerechtfertigt ist, benötigen klinisch apparente oder wachsende Zysten eine Therapie. Bei symptomatischen Zysten ist eine laparoskopische Zystenentdachung die Therapie der Wahl. Komplizierte Fälle können eine offene Zystenentdachung oder eine Leberresektion benötigen. Eine Aspiration mit Sklerotherapie wird wegen der hohen Rezidivrate nur für Patienten erwogen, die absolute Kontraindikationen für eine operative Versorgung aufweisen.
