Fieber in der Intensivmedizin
Fieber ist ein sehr häufiges, dabei unspezifisches Symptom auf der Intensivstation, kann ganz verschiedene Ursachen haben und ist assoziiert mit einem schlechteren Outcome. Insofern muss jedes Fieber auf der Intensivstation abgeklärt werden, und das Temperaturmanagement richtet sich nach Grunderkrankung, klinischem Zustand des Patienten bzw. der Patientin und evtl. unerwünschten Wirkungen der Therapiemaßnahmen.

Fieber auf der Intensivstation ist sehr häufig.

Fieber ist definiert als eine Erhöhung der Körperkerntemperatur über den tageszeitabhängigen Normbereich einer Person hinaus; es gibt also keinen allgemein anerkannten Schwellenwert für Fieber.

Fieber ist ein unspezifisches Symptom und kann verschiedene infektiologische oder nicht infektiologische Ursachen haben.

Prolongiertes Fieber und hohe Temperaturen gehen mit einer erhöhten Mortalität einher.

Fieber ist assoziiert mit:

längerer Intensivbehandlungsdauer,
-
höheren Behandlungskosten,
-
schlechterem Outcome.

Jedes Fieber auf der Intensivstation muss abgeklärt werden!

Methodenimmanente Unterschiede der verschiedenen Messverfahren müssen beachtet werden. Zur Diagnostik gehören des Weiteren eine fokussierte Anamnese, die körperliche Untersuchung, Labordiagnostik (Blutkulturen) und ggf. Bildgebung.
Temperaturmanagement auf der Intensivstation ist ein kontrovers diskutiertes Thema. Neben pathophysiologischen Überlegungen, der Symptomatik des Patienten und potenziell unerwünschten Wirkungen der Antipyretika entscheidet vor allem die Grunderkrankung über das Temperaturmanagement.
