Lokal fortgeschrittene Kopf-Hals-Tumoren: Therapiekonzepte und neue Ansätze
Lokal fortgeschrittene Kopf-Hals-Tumoren stellen aufgrund ihrer oft kritischen Lage bzgl. Funktionalität und Lebensqualität ein therapeutisches Problem dar, welches einer engen interdisziplinären Zusammenarbeit bedarf. Im folgenden Artikel möchten wir den Lesern eine Übersicht über die aktuellen Therapieprinzipien, über aktuelle radioonkologische und operative Technik sowie über den aktuellen Stand zur Immuntherapie im kurativen Setting geben.

Plattenepithelkarzinome des Kopf-Hals-Bereiches stellen bzgl. ihrer Lokalisation, Ausdehnung und Prognose eine heterogene Gruppe von Erkrankungen dar.

Die kurative Therapie erfolgt interdisziplinär und oft multimodal mit den tragenden Säulen der operativ-rekonstruktiven Chirurgie und der Strahlentherapie in Kombination mit einer Systemtherapie.

Die Inzidenz Virus-assoziierter Tumoren (insbesondere HPV) steigt in westlichen Ländern an. Aufgrund ihrer guten Prognose laufen klinische Studien, die eine sichere Therapiedeeskalation zur Toxizitätsreduktion untersuchen und deren Ergebnisse in den nächsten Jahren erwartet werden.

Bei der Therapieentscheidung müssen neben dem onkologischen Outcome auch die mögliche Lebensqualitätseinschränkung und die Spättoxizitäten der Behandlungsmöglichkeiten mit dem Patienten erörtert werden.

Im letzten Jahrzehnt haben Neuerungen in der Bestrahlungsplanung, Applikationstechnik und bessere Modelle zur Toxizitätsabschätzung zu einer deutlichen Verbesserung der Lebensqualität nach Radiotherapie geführt.

Die Immuntherapie hat bislang keinen Stellenwert im kurativen Setting bei Kopf-Hals-Tumoren der Schleimhaut; verschiedene Ansätze in der neoadjuvanten oder adjuvanten Behandlung werden aber untersucht.
