Therapie der physischen Frailty
Therapy of physical frailtyFrailty beschreibt als ein übergeordnetes geriatrisches Syndrom ein potenziell reversibles Übergangsstadium zwischen funktioneller Autonomie und irreversibler Hilfsbedürftigkeit. Frailty adressiert damit ein „Window of Opportunity“, in dem funktionelle Einschränkungen mit Erfolg therapiert werden können. Der vorliegende Beitrag gibt einen Überblick über aktuelle Therapieansätze und ihre wissenschaftliche Evidenz.
Abstract
As an overarching geriatric syndrome, frailty describes a potentially reversible transitional stage between functional autonomy and irreversible disability. Thus, frailty addresses a “window of opportunity” in which functional limitations can be successfully treated. This article provides an overview of the therapeutic approaches and their scientific evidence.

Ziele der Frailty-Therapie sind die Verbesserung der funktionellen Autonomie und der Lebensqualität.

Die „First-Line-Therapie“ ist ein körperliches Mehrkomponententraining, bestehend aus Kraftübungen, kombiniert mit anderen Trainingsmodalitäten (Balance, Ausdauer, funktionelles Training).

Die allgemeinen Empfehlungen der WHO zur körperlichen Aktivität gelten auch für ältere Personen mit Frailty: 150 Minuten pro Woche mit moderater Intensität.

Bei Vorliegen einer Protein-Energie-Malnutrition sollte die Trainingstherapie zwingend mit einer Ernährungstherapie kombiniert werden.

Die Therapie beteiligter Komorbiditäten und ein (Multi-)Medikationscheck mit Blick auf sturz- oder delirfördernde Arzneimittel sollten fester Bestandteil der Therapie sein.
