Die Entzugsbehandlung von Gamma-Hydroxybuttersäure (GHB) im stationären Setting

Trotz eines relativ weit verbreiteten und zunehmenden Konsums von Gamma-Hydroxybuttersäure in der Partyszene gibt es nur wenige Berichte über die Behandlung der GHB-Abhängigkeit – meist mittels hoch dosierter Gabe von Benzodiazepinen nach einem individuellen Schema. Klare Behandlungsempfehlungen im Sinne einer Leitlinie liegen bisher nicht vor.
Kernaussagen

  • Auch wenn die Prävalenz eines GHB-Missbrauchs vergleichsweise gering ist, sollte bei Patienten mit unklarer Mischintoxikation mit sedierenden Substanzen aufgrund der ausgeprägten und nicht selten vital bedrohlichen Entzugssymptomatik GHB als ursächlich in Betracht gezogen werden.

  • Der Entzug von GHB erfolgt üblicherweise mittels hoch dosierter Gabe von Benzodiazepinen nach einem individuellen Schema, wobei eine Therapieresistenz mit dem Risiko der Entwicklung eines Delirs vorkommen kann.

  • Der Einsatz medizinischem GHBs kann im Rahmen der Entzugsbehandlung das Auftreten komplizierter Verläufe reduzieren.

  • Eine ambulante Entzugsbehandlung sollte aufgrund der oftmals unklaren Menge konsumierten GHBs und der Möglichkeit schwerwiegender Komplikationen möglichst vermieden werden.

  • GHB wird in der Party-Szene häufig als „date rape drug“ verwendet, insbesondere durch die Geschmack- und Farblosigkeit der Substanz und ihrer kurzen Nachweisbarkeit in Blut und Urin über wenige Stunden. Zudem ist Missbrauch innerhalb der Chemsex-Szene häufig. Gerade bei diesen Risikogruppen sollten deshalb präventive psychoedukative Maßnahmen umgesetzt werden.
