Analgosedierung in der Intensivmedizin

Die Analgosedierung intensivmedizinischer Patienten ist eine im klinischen Alltag regelhaft auftretende, komplexe Aufgabe. Es gilt, dem individuellen Patienten das für ihn am besten geeignete Medikament so zu verabreichen, dass eine effektive Wirkung erreicht und unerwünschte Nebenwirkungen minimiert werden. Dieser Artikel soll eine Übersicht über die Ziele der kontinuierlichen Sedierung sowie die aktuell verfügbaren, gängigen Pharmaka vermitteln.

Kernaussagen
  • Ein der individuellen Situation des Patienten angemessenes Sedierungsregime kann maßgeblich zum Genesungsprozess des Patienten beitragen.

  • Das regelmäßige Management der Sedierungstiefe anhand valider Scores stellt einen Qualitätsindikator der intensivmedizinischen Therapie dar.

  • Zum Erreichen einer adäquaten Sedierung steht eine Vielzahl an Pharmaka zur Verfügung.

  • Bei der Auswahl der Pharmaka sollte sowohl auf einen ausreichenden sedierenden als auch analgetischen Effekt geachtet werden.

  • Spezielle Patientengruppen erfordern ein angepasstes Sedierungsregime.
