Paola Soriani et al. Over-the-scope clip as first-line treatment of peptic ulcer bleeding: a multicenter randomized controlled trial (TOP Study)

Primary therapy with over-the-scope (OTS) clips versus through-the-scope clips for bleeding peptic ulcers was investigated in this multicenter randomized trial. In 112 patients with bleeding peptic ulcers, OTS clips demonstrated a significantly higher rate of initial success and overall clinical success for endoscopic hemostasis. While the results suggest routine use of OTS clips as a primary hemostatic method for bleeding peptic ulcers, this should be carefully balanced against the consideration of cost-effectiveness.
