The added value of a computer-aided detection (CADe) system for adenoma detection was assessed in this randomized, multicenter trial performed in Europe and Canada. Data from 497 patients undergoing diagnostic, screening, or surveillance colonoscopy did not show any significant difference in the adenoma detection rate or adenomas per colonoscopy between CADe-assisted colonoscopy and conventional colonoscopy. The number of sessile serrated lesions per colonoscopy was increased with CADe compared with conventional colonoscopy. This study raises concerns about the efficacy of a CADe sytem in improving adenoma detection among a varied, everyday colonoscopy population.
