Adenoma detection rate (ADR) is a key quality indicator in colonoscopy; however, data are often derived from prospective studies for selected indications or randomized controlled trials. In “real world” practice, indications and patient groups may vary. This review examined data from outside of carefully controlled studies to estimate the ADR in direct colonoscopy screening (27%), post-fecal immunochemical testing (34%), surveillance (43%), diagnostic (25%), and overall (27%) groups. These results suggest that current society guideline recommendations for a baseline ADR of 25% are appropriate.
