We read with great interest the article by Truss et al[1] regarding their single-surgeon experience with 21 cases of the Universal 2® total wrist prosthesis. The authors reported a 10-year survival rate of 68% with the same prosthesis. They questioned the difference with 92% of the 10-year survival rate reported in a multicenter, multisurgeon study involving 69 cases.[2]
Our own single-surgeon experience with 23 cases of another prosthesis, the Remotion®, corroborates the findings of Truss et al.[1] We observed a 10-year survival rate of 69%,[3] whereas a multicenter, multisurgeon study involving 65 cases reported a 9-year survival rate of 90% with the same prosthesis.[4]
In these various studies, the proportions of patients with rheumatoid arthritis versus osteoarthritis were generally comparable. Therefore, the type of prosthesis does not appear to explain the variations in survival rates but rather other factors such as whether the study was single-center or multicenter, the sample size, or the surgeons' level of experience.
These observations highlight the need for the establishment of an international registry dedicated to objectively and scientifically evaluating the performance of total wrist prostheses.
We thank Truss et al for their very interesting article, which underscores the importance of comparing the outcomes of different series of wrist prostheses.
