Zusammenfassung
Die Langzeitbetreuung nach tiefer Hirnstimulation sollte sich nicht auf die technische Kontrolle des implantierten Stimulationssystems und der Beobachtung des Energieverbrauchs zur rechtzeitigen Planung eines nötigen Batteriewechsels beschränken. Verschlechtert sich der Zustand des Patienten, muss geklärt werden, ob eine Zweiterkrankung, eine Fehlfunktion des Stimulationssystems oder die natürliche Krankheitsprogression auslösend ist. Zusätzlich sollten übende Therapieverfahren angeboten werden, denn Patienten mit fixierten unphysiologischen Bewegungsmustern können von speziellen rehabilitativen Maßnahmen profitieren. Psychosoziale Adaptationsprobleme bedürfen der gezielten psycho- und familientherapeutischen Unterstützung. Im Folgenden werden die relevanten Aspekte der Stimulation sowie der multiprofessionellen Betreuung dargestellt, die im Rahmen strukturierter Nachsorgeprogramme zur Zufriedenheit der Patienten und ihres sozialen Umfeldes beitragen können.
Abstract
Long-term care after deep brain stimulation for Parkinson's disease should not only focus on a technical check of the implanted device and monitoring of battery drain. If the patient's condition deteriorates, malfunctioning of the stimulation system, comorbidity or natural progression of the underlying disease has to be considered and treated accordingly. Patients with fixed unphysiological movement patterns may benefit from rehabilitative programmes. Psychosocial maladjustment may call for psychotherapy or family counselling. Relevant aspects of stimulation as well as multiprofessional treatment modalities are presented, which may contribute to long-term patient satisfaction.
