La recherche d'une dilatation ventriculaire compliquant la rupture d'un anévrysme artériel sus tentoriel s'est révélée positive dans 42,8% des cas sur un total de 91 malades examinés par angiographie carotidienne (index ventriculaire supérieur à 0,25).
La dilatation ventriculaire aiguë est observée dans les trois jours qui suivent la rupture (18 cas). C'est le blocage des citernes ou de l'aqueduc de Sylvius qui en sont responsables surtout si des hémorragies méningées antérieures ont entraîné un cloisonnement partiel des espaces sous arachnoidiens. Ces dilatations indiquent une ponction ventriculaire d'urgence suivie d'un drainage aseptique qui a pu faire régresser un coma et permettre un abord direct de la lésion.
Les hydrocéphalies secondaires installées après le dixième jour (21 cas) sont mieux connues et leur diagnostic est précisé par la prise de pression intra-ventriculaire comme par le transit isotopique, qui indiqueront ou non la mise en place d'une dérivation permanente ventriculo-atriale.
Ces troubles de la capacité ventriculaire paraissent être favorisés par le siège de l'anévrysme (les dilatations sont plus fréquentes dans les variétés médianes), par la diminution du débit circulatoire cérébral qu'il soit dû à un spasme (60%), à un hématome intra-cérébral (54%) ou à un terrain a théromateux, ces différents facteurs agissant sans doute par ischémie périventriculaire.

Bei 91 Fällen von rupturierten supratentoriellen Aneurysmen wurde in 42,8 % Erweiterung der Ventrikel mit Hilfe der Karotisangiografie nachgewiesen (Ventrikelindex größer als 0,25 %).
Eine akute Ventrikelerweiterung erschien innerhalb der ersten Tage nach der Ruptur (18 Fälle). Dieser Hydrocephalus wurde durch die Tamponade der basalen Zisterne oder des Aquäduktes insbesondere dann gefunden, wenn die Verletzungen die Subarachnoidalräume ausfüllten. Diese akuten Erweiterungen erfordern eine intrazerebrale Punktion mit anschließender Drainage. So ist ein Koma rückbildungsfähig und ermöglicht eine Operation des Aneurysma.
Sekundäre Hydrocephali entwickeln sich nach dem 10. Tag (21 Fälle). Sie wurden durch intraventrikuläre Druckmessung und durch RISA-Cisternographie dokumentiert. Diese Untersuchungen sind die Indikationen für einen ventrikuloatrialen Shunt. Die Komplikationen von seiten der Ventrikel scheinen vom Sitz des Aneurysmas abhängig zu sein; Erweiterungen sind häufiger bei Aneurysmen der Mittellinie durch Verminderung der cerebralen Durchblutung infolge eines arteriellen Spasmus (60 %), durch intrazerebrale Hämatome (54 %) oder durch eine Arteriosklerose, die eine periventrikuläre Ischämie hervorrufen, wie dies experimentell nachgewiesen werden konnte.
Summary
Among 91 cases of ruptured supra-tentorial arterial aneurysms 42.8 % of ventricular enlargement were diagnosed by carotid angiographic studies (ventricular index greater than 0.25).
Acute ventricular dilatation appeared during the three days after rupture (18 cases). This hydrocephalus is caused by the blood obstructing the basal cisterns or the aqueduct of Sylvius, particularly if anterior haemorrhages have involved a partial closing off of the subarachnoid pathways. These acute dilatations demand an urgent ventricular tap, followed by drainage. In this way a coma is reversible and will permit a direct approach to the aneurysm.
Secondary hydrocephalus developed after the tenth day (21 cases) and the different varieties are shown by an intra-ventricular recording of the pressure as well as by RISA cisternography. These examinations may or may not suggest the necessity for a permanent ventriculo-atrial shunt.
The complications of ventricular enlargement seemed to be favoured by the site of the aneurysm, (dilatations are more frequent when aneurysms are in the midline), by the drop in cerebral blood-flow induced by arterial spasm (60 %), by intra-cerebral haematoma (54 %) or by atheromatous arteriopathy provoking a peri-ventricular ischaemia as shown by experimental work.
