Summary
To begin with, we would like to summarize the surgical technique employed in bilateral, usually quite large tumors of the cerebello-pontine angle as follows:

Positioning of the patient in a sitting posture on the operating table.

Unilateral craniotomy. The side affected by the larger of the tumors should be attacked first.

Extirpation of the contralateral tumor as soon after the initial intervention as possible (2 to 3 weeks). This is essential to avoid persistent and/or increasing unilateral compression.

Approach to the tumor proper:
-
Intracapsular dissection of the tumor.
-
Decompression of the brain stem by detaching the tumor from it.
-
Search for facial nerve in the area of the medial tumor portion. The nerve may lie above the tumor; rostral or caudal sites are also possible. Inspection of the capsule from outside often does not reveal the intracapsular course of the nerve. The capsule color does not serve as a distinguishing sign. If identification on the brain stem and on the upper and lower surface of the tumor capsule is unsuccessful, we try to locate the nerve by gentle probing from the interior of the emptied capsule.
Tracing of the nerve laterally towards the porus acusticus internus is not done without prior exposure and identification of the facial trunk (by electrostimulation, if necessary).
After separation of the nervous and vascular structures from the capsule, the latter is resected piecemeal.
After resection of the posterior porus and meatal walls by means of the diamond drill the meatal tumor portion is extirpated. Short of its entry into the porus, the facial nerve is often found to be flattened into a thin membrane.
Only in small tumors should the facial nerve, after emptying of the tumor capsule, first be identified at the porus and then traced towards the brain stem.

After completing the intracranial intervention the dura should be closed by grafting with lyophilized dural material.
Zusammenfassung
Einleitend möchten wir die von uns bei beidseitigen zumeist extrem großen Tumoren der Kleinhirnbrückenwinkel angewandte Operationstechnik in folgenden Punkten zusammenfassen:

Lagerung des Patienten in sitzender Stellung.

Unilaterale Trepanation, zuerst auf der Seite des größeren Tumors.
-
Exstirpation des Tumors der Gegenseite möglichst bald nach der Erstoperation (2–3 Wochen) um den Hirnstamm vom nun einseitigen Druck zu befreien.

Eingriff am Tumor selbst (alle Tumoren über 4 cm Durchmesser):
-
intrakapsuläres Aushöhlen des Tumors.
-
Entlastung des Hirnstamms durch Abpräparation des Tumors.
-
Suche nach dem N. facialis im Bereiche der medialen Tumorhälfte. Der Nerv kann über dem Tumor, rostral oder caudal von ihm, liegen. Bei der Betrachtung der Kapsel von außen ist der Nerv durch seinen intrakapsulären Verlauf oft nicht zu sehen; er unterscheidet sich auch in der Farbe nicht von der Kapsel. Wir suchen daher den Nerven, wenn wir ihn am Hirnstamm und im Bereiche der Ober- und Unterfläche der Tumorkapsel nicht identifizieren können, durch vorsichtiges Präparieren von der Innenfläche des ausgehöhlten Tumors aus auf.
Erst nach Darstellung und Identifizierung des Nervenstamms (ev. Elektrostimulation) wird der Nerv nach lateral zum Porus acusticus internus verfolgt.
Der im Meatus liegende Tumoranteil wird nach Entfernung der Hinterwand des Porus und Meatus mittels Diamantbohrers exstirpiert.
Vor dem Eintritt in den Porus sieht man den N. facialis oft zu einer dünnen Membran ausgespannt.
Nur bei kleinen Tumoren kann man nach Aushöhlung des Tumors den N. facialis zuerst am Porus aufsuchen und ihn dann nach medial zum Hirnstamm präparieren.

Nach Beendigung des intrakraniellen Eingriffes plastischer Duraverschluß mittels lyophilisierter Dura.
Resumé
Pour conclure, nous pourrions résumer comme suit la technique chirurgicale des tumeurs bilatérales de l'angle pontocérébelleux, de volume habituellement considérable:

Intervention en position assise.
-
Trépanation unilatérale du côté de la tumeur la plus volumineuse.
-
Extirpation de la tumeur contra-latérale le plus tôt possible après la première intervention (2 à 3 semaines) ce qui est essentiel pour éviter la persistance ou l'aggravation d'une compression unilatérale.

Pour accéder à la tumeur elle-même:
-
Dissection intracapsulaire.
-
Séparation de la tumeur du tronc cérébral en vue de le décomprimer.
-
recherche du nerf facial du côté interne de la tumeur. Il peut être situé audessus d'elle; des positions antérieures ou postérieure étant également possibles. L'inspection de la capsule de l'extérieur trahit rarement un trajet intracapsulaire du nerf. Sa coloration n'en donne pas un signe indicateur. Si l'investigation du tronc cérébral, des parties supérieure et inférieure de la capsule est restée négative, nous recherchons à localiser le nerf par voie interne après son curetage. La poursuite du nerf facial vers le trou auditif interne suppose son identification préalable au besoin par stimulation électrique. La capsule est réséquée à la pince emportepièce après son clivage des formations nerveuses et vasculaires. Après mise en évidence du trou auditif interne et de ses parois, la portion de la tumeur qui l'occupe est éliminée à la fraise à diaments. A peine a-t-il franchi le trou auditif, le nerf facial est souvent réduit à une mince lamelle. Ce n'est qu'en cas de tumeurs de petit volume que le nerf facial pourra être identifié au trou auditif et de là suivi du côté interne vers le tronc cérébral après curetage de la capsule.

A la fin de l'intervention intracranienne la brèche duremérienne devrait être obturée par une greffe de dure-mère lyophilisée.
