Abstract
A case of a 4-9/12 years old boy with congenital fibre type disproportion is presented. The clinical picture consisted of severe muscular hypotonia and weakness, areflexia, underweight and kyphoskoliosis. Intercostal weakness, leading to cyanosis during sleep and repeated lung infections, necessitated tracheostomy and artificial respiration for periods of increasing length.
Muscle biopsy revealed selective hypoplasia and predominance of type 1 fibres, normal or large type 2 A fibres and absence of 2 B fibres. EMG showed a reduced interference pattern with complex high amplitude unit potentials.
The relationship between congenital fibre type disproportion and nemaline or centronuclear myopathy with type 1 fibre atrophy is discussed.
Zusammenfassung
Es wird über einen 49/12 Jahre alten Jungen mit angeborener schwerer Muskelhypotonie, Schwoäche, Areflexie, Untergewicht und Kyphoskoliose berichtet. Zunehmende Schwachc der Interkostalmuskulatur führte zu Zyanose im Schlaf und zu rezidivierenden Pneumonien, die eine Tracheotomie und Beatmung während des Schlafes erforderlich machten. Die Muskclbiopsie zeigte eine selektive Hypoplasie und relative Vermehrung der Typ 1-Fasern, normale oder vergrößerte Fasern von Typ 2A und fehlende 2 B-Fasern. Im EMG war das Interferenzmuster gelichtet, die Einzelpotentiale waren komplex, hochamplitudig und teilweise verbreitert.
Der Fall entspricht dem Krankheitsbild, das von Brooke als angeborene Disproportion der Fasertypen (congenital fibre type disproportion) beschrieben wurde. Eine scharfe Abgrenzung dieser Erkrankung gegeniiber Fallen von Nemalinmyopathie oder zentronuklearer Myopathie mit Typ 1-Faser-Atrophie ist kaum moglich.
