Abstract
Attention is drawn to the occurrence of spinal cord damage due to birth injury. The findings in two infants are reported who died at age 3 and 7 weeks, respectively, as the result of such injuries at the cervical and low-thoracic levels after breech delivery and manual extraction. The clinical features were flaccid tetraparesis and generalized muscle atrophy with hyperreflexia at the lower limbs, and flaccic paraparesis of the legs, associated with breathing disorders. In both infants, neurological symptoms occurred immediately after delivery. At autopsy, there was destruction of the cord over several segments with focal cicatrication, consisting largely of hypervascularized connective tissue, and extensive adherence of the grossly thickened and discolored meninges. In case 2 post-traumatic vasopathy with occlusion of spinal arteries at T-8 resulted in additional ischemic necrosis of the lumbar gray matter. Skeletal muscle in case 1 showed neurogenic atrophy of the spinal type which was consistent with the neurophysiological findings. These facts should be considered in the differential diagnosis of neonatal spinal cord injury which includes infantile spinal muscular atrophy. The pathology of the late stages of spinal cord birth injury does not significantly differ from those in the adult.
Zusammenfassung
Klinisch-morphologischer Bericht über mechanisch-geburtstraumatische Rükkenmarksschäden im Hals- bzw. kaudalen Thorakalmark nach Steißgeburt bzw. Manualextraktion bei zwei Säuglingen, die im Alter von 7 bzw. 3 Wochen verstarben. Klinisch bestanden sofort nach der Entbindung schlaffe Tetrabzw. Paraparesen und Atemstörungen. Das Mädchen mit Halsmarkläsion (Fall 1) entwickelte generalisierte Muskelatrophien und bot neurophysiologische Zeichen von spinaler Vorderhornschädigung. Bei dem Knaben mit Brustmarkläsion (Fall 2) lag ein Sperrliquor vor. Beide Fälle boten pathologische Wirbelröntgenbefunde. Autoptisch fand sicheinemehrsegmentale Zerstörungdes Rückenmarks mit umschriebener, vorwiegend bindegewebiger Vernarbung unter Einbeziehung der mächtig verschwarteten Rückenmarkshaüte. Eine posttraumatische Vasopathie mit Verschluß der vorderen Spinalarterie führte im Fall 2 zu ischämischen Nekrosen im lumbalen Grau. Der Skelettmuskel im Fall 1 bot das Bild einer neurogenen Atrophie vom spinalen Typ, was mit den EMG-Befunden im Einklang steht. Diese Daten scheinen von Interesse für die Differentialdiagnose geburtstraumatischer Rückenmarksschäden, die u. a. auch die WERDNIG-HOFFMANNsche Krankheit einschließt.
