Abstract
Four cases with lumbosacral dysrhaphia are described which were initially incorrectly evaluated and were only treated adequately after a delay. In connection with these cases the diagnostic significance of different symptoms of dysrhaphia are presented and the subsequent therapeutic measures are discussed.
Spina bifida occulta in the lumbosacral area is very frequently encountered as a normal variant. Relatively seldom it is a sign of dysrhaphia which includes additional morphological abnormalities of the spinal cord, the nerve roots, the meninges or the overlying skin. Statistically there is no correlation between spina bifida occulta and enuresis.
Congenital dermal sinuses are superficially located in the majority of cases; occasionally they extend to the intraspinal space and may terminate outside or inside the dura or in the cord itself. They frequently lead to extradural, subdural or intramedullary abscesses and to recurring bacterial meningitis. Dermal sinuses which on palpation are connected with the interspinal ligament or complicated by local infections, by meningitis or abnormal neurological signs must be completely removed. Dermal sinuses may be combined with dermoid cysts and/or lipomas.
Dermoid cysts and lipomas are located subcutaneously or intraspinally, extradurally or intra-durally, inside or outside the spinal cord. The great majority of them are associated with spina bifida occulta. Infected dermoid cysts may enlarge rapidly and if located intraspinally may lead to a syndrome of acute spinal compression. Subcutaneously located dermoid cysts should always be removed as soon as possible.
Hypertrichosis, naevus vasculosus and pigment anomalies of the skin over the spine require a clinical and radiological examination of the underlying part of the spine and thorough neurological examination especially of the lower half of the body.
Zusammenfassung
Es werden 4 Fälle mit lumbosacraler Dysrhaphie mitgeteilt, die zuerst falsch beurteilt und verspäter einer adäquaten Therapie zugeführt wurden. Im Zusammenhang damit wird die diagnostishe Bedeutung einzelner Symptome der Dysrhaphie und der sich daraus ergebenden therapeutischen Konsequenzen Kurz dargestellt.
Die lumbosacrale Spina bifida occulta tritt sehr häufig als Normvariante ohne, relativ selten als dysrhaphisches Symptom mit zusätzlichen morphologischen Veränderungen von Rückenmark, Nervenwurzeln, Meningen oder Haut und Subcutis auf. Es besteht keine statistische Korrelation zwischen Spina bifida occulta und Enuresis.
Congenitale Dermalsinus und Dermalfisteln liegen meist oberfiächlich, d. h. extraspinal; gelegentlich reichen sie jedoch bis in den Intraspinalraum und können epidural, subdural oder intramedullär enden. Sie führen häufig zu rezidivierenden Infekten, nämlich zu epiduralen, subduralen oder intramedullären Abszessen und zu eitriger Meningitis. Palpatorisch am Ligamentum interspinosum fixierte oder durch Infekte, Fremdkörperwirkung oder neurologische Ausfallserscheinungen komplizierte Dermalsinus sind total zu exstirpieren. In einem Teil der Fälle sind Dermalsinus und Dermalfisteln mit einer Dermoidcyste und/oder einem Lipom kombiniert.
Dermoidcysten und Lipome liegen subcutan oder intraspinal-extradural oder intradural-extramedullär oder intramedullär. Sie sind meist, aber nicht immer mit einer Spina bifida occulta kombiniert. Bei Infektion einer Dermoidcyste kann diese rasch an Größe zunehmen und bei intraspinaler Lage zu einem akuten spinalen Kompressionssyndrom führen. Subcutane Dermoide und Lipome sollen immer möglichst frühzeitig exstirpiert werden.
Hypertrichose, Naevus vasculosus und Pigmentanomalien in der dorsalen Mittellinie erfordern eine palpatorische und eine radiologische Untersuchung des darunterliegenden Wirbelsäulenabschnittes sowie eine neurologische Untersuchung insbesondere der unteren Körperhälfte.
