Abstract
A three months-old baby showed a syndrome of acute subdural hematoma with consecutive internal hydrocephalus and sclerotic brain atrophy. Angiographic and pmeumographic studies and the post mortem findings indicate : 1. the dynamic communicating hydrocephalus is the result of CSF-resorption deficit caused by the subdural hematoma; 2. the sclerotic brain atrophy is the effect of the intracranial hypertension (subdural effusion and intraventricular hypertension) and of a transitory impairment of blood supply being proved by ulegyrias. Because the ulegyrias were found in cerebral and cerebellar specimen, an extracranial factor is supposed to have been the cause for the parenchymal lesions and for the subdural hematoma. Additionally observations were made concerning the secondary development of the so-called pachymeningeosis haemorrhagica interna during the chronic phase of the subdural effusion.
Zusammenfassung
Ein drei Monate alter Säugling bietet ein Syndrom von akutem subduralem Hämatom mit nachfolgendem Hydrocephalus internus und sklerosierender Hirnatrophie. Die angiographischen, pneumographischen und späteren autoptischen Befunde lassen annehmen:
1. der dynamische kommunizierende Hydrocephalus ist Folge eines Liquorresorptionsdefizits, welches durch das subdurale Hämatom verursacht ist; 2. die sklerosierende Hirnatrophie ist Folge sowohl des intracraniellen Überdrukkes (raumfordernder, subduraler Erguß und intraventrikulärer Überdruck) wie auch einer vorübergehenden Beeinträchtigung der Blutversorgung, die anhand von Ulegyrien nachzuweisen ist. Weil die Ulegyrien im Großhirn und Kleinhirn vorhanden sind, wird auf einen extracraniellen Faktor geschlossen, der die Hirnparenchymschäden, gleichzeitig auch das subdurale Hämatom verursacht hat. Zusätzlich wurden Beobachtungen über die sekundäre Entstehung der sogenannten Pachymeningeosis haemorrhagica interna gemacht.
