Endoscopic-bioptical Diagnosis of Antral Mucosa within the Duodenal Stump

Summary

In a 26-year-old man an anastomotic ulcer developed following Billroth II resection. Endoscopic intubation of the afferent loop was successful with bioptical demonstration of a retained antrum in the duodenal stump. Since only by this method the pathogenesis of recurrent ulceration in an operated stomach may be verified biopsy of the duodenal stump should be performed in all stomal ulcers.

Zusammenfassung

Bei einem 26jährigen Mann entwickelte sich ein Anastomosenulkus nach einer Billroth-II-Resektion. Es gelang, das Endoskop in die zuführende Schlinge einzuführen und bioptisch einen Antrumrest am Duodenalstumpf nachzuweisen. Da diese spezifische Pathogenese eines Ulkus nach Magenresektion nur mit dieser Methode eruiert werden kann, sollte eine bioptische Untersuchung des Duodenalstumpfes bei allen Fällen mit Anastomosenulkus ausgeführt werden.
