Summary
The fibrorectosigmoidoscope Olympus TCF I S, has been systematically applied to the exploration of the distal colon (rectum and sigmoid) in patients with intestinal or digestive extra intestinal symptoms. In the first 300 patients the technical parameters of the method were tested: the fleet enema preparation was found excellent or satisfactory in 95 % of examinations. The tolerance was good in 96 % of examinations — the mean length of operation was 5.15 min — the mean length of colon examined was 48.5 cm, the distance of 50 cm being surpassed in 50 % of patients. The efficacy in the detection of the lesions was tested in 476 patients. Tumoral colonic lesions were detected in 105 patients with a total of 125 adenomas and 31 cancers. In the area that could have been explored by a rigid rectosigmoidoscope (0—19 cm) we found 71 lesions (50 adenoma and 10 carcinoma), whereas beyond this limit (25—60 cm) 85 lesions (50 adenoma and 10 carcinoma) were found. The flexible rectosigmoidoscope explores the rectum and the full length of sigmoid resulting in greater efficacy in the detection of colonic lesions. Therefore, the flexible rectosigmoidoscope should replace the rigid instrument within a short time.
Zusammenfassung
Das Fiberrektosigmoidoskop TCF I S von Olympus wurde zur Untersuchung des distalen Kolons bei entsprechenden Beschwerden gezielt eingesetzt. Bei den ersten 300 Patienten wurden die technischen Details analysiert: ein Fleet-Einlauf erwies sich bei 95 % der Untersuchungen als ausreichend, die Untersuchung wurde von 96 % der Patienten bei einer Untersuchungszeit von durchschnittlich 5,15 min und einer Strecke von 48,5 cm gut toleriert. Bei rund der Hälfte der Patienten konnte das Instrument mindestens 50 cm weit hochgeschoben werden. Bei 476 Patienten wurde nach umschriebenen tumorösen Schleimhautveränderungen gesucht. Bei 105 Patienten wurden insgesamt 125 Adenome und 31 Karzinome gefunden. Im Bereich des starren Rektosigmoidoskops (0—19 cm) fanden sich 71 Befunde (50 Adenome und 10 Karzinome), zwischen 25 und 60 cm 85 Läsionen (50 Adenome und 10 Karzinome). Aus Gründen der größeren Reichweite und einer damit erhöhten Ausbeute sollte das flexible Rektosigmoidoskop bald das starre Instrumentarium ersetzen.
