Summary
Presently endoscopic examination is the single most important diagnostic procedure for the early recognition and evaluation of bladder tumors. The cystoscope has a particularly valuable function in evaluation of patients suspected of having carcinoma-in-situ since the diagnosis can only be secured by bladder biopsies under vision in suspected areas. Recently, the Cytologic examination of urine samples has received increased importance in these patients since the carcinoma in situ lesions are difficult to visualize, especially early in the disease process. There are studies in progress with the aim to improve UV light cystoscopy techniques that in the past have been of equivocal help diagnostically. Because of early lymphogenous spread of the tumor even very careful endoscopic examination can lead to underestimation of the extension of the tumor. Understaging varies between 7 and 74% depending on the extent of the tumor. A final staging is possible only by histologic examination of the dissected surgical specimen, including the pelvic lymph nodes. The exact knowledge of the tumor extension is a fundamental prerequisite for rational and appropriate treatment of the individual patient. According to our present knowledge transurethral electro-resection of bladder tumors has the potential for cure only in stages 0, A and B2 and radical cystectomy is generally reserved for stages B2 and C. Interval cystoscopy is absolutely mandatory in these patients throughout their lives since the tendency for recurrence, even in low stage lesions, is high in bladder tumor patients.
Zusammenfassung
Gegenwärtig ist die endoskopische Untersuchung die entscheidende diagnostische Maßnahme zur Früherkennung und zur Beurteilung von Harnblasentumoren. Die Zystoskopie spielt eine besondere Rolle bei den Patienten, bei denen ein Carcinoma in situ vermutet wird, da die Diagnose nur durch Biopsien unter Sicht aus den verdächtigen Bezirken der Harnblase gesichert werden kann. In jüngster Zeit hat die zytologische Untersuchung von Urinproben bei diesen Patienten an Bedeutung gewonnen, da die Veränderungen beim Carcinoma in situ vor allem im Frühstadium der Erkrankung häufig zystoskopisch nicht zu erkennen sind. Es wird zur Zeit versucht, die UV-Licht-Zystoskopie, die bislang nur einen zweifelhaften diagnostischen Wert hatte, technisch zu verbessern. Infolge der frühzeitigen lymphogenen Metastasierung der Tumoren kann selbst eine sehr sorgfältige endoskopische Untersuchung zu einer Unterschätzung der Tumorausdehnung führen. Diese variiert in Abhängigkeit vom Tumorstadium zwischen 7 und 74%. Eine endgültige Stadieneinteilung ist nur nach histologischer Untersuchung des Operationspräparates einschließlich der Beckenlymphknoten möglich. Die genaue Kenntnis der Tumorausdehnung ist eine Grundvoraussetzung für die angemessene Behandlung jedes einzelnen Patienten. Nach dem gegenwärtigen Wissensstand können mit der transurethralen Elektroresektion nur die Harnblasentumoren der Stadien O, A und B1 potentiell geheilt werden, während die radikale Zystektomie den Stadien B2 und C vorbehalten bleibt. Lebenslange, regelmäßige zystoskopische Kontrolluntersuchungen sind bei diesen Patienten unerläßlich, da die Rezidivneigung selbst bei Tumoren der Frühstadien sehr groß ist.
