Summary
Compared with colonic polyps gastric polyps may show a histological structure which has no analogon in the gut. This gastric-specific polyp is called hyperplasiogenous since it exhibits hyperplastic as well as adenomatous structures. Malignant degeneration of this polyp is unlikely. The malignant polyp of the stomach is an early carcinoma type I which most probably originates from a proliferation of the glandular neck with cellular atypia (borderline lesion protruded type). With the common occurence of multiple hyperplasiogenous polyps the common identification of polyposis and precancerosis has to be revised.
Zusammenfassung
Im Vergleich zu den Polypen des Dickdarms läßt sich bei der Klassifikation der Magenpolypen nach der histologischen Grundstruktur ein besonderer Typ herausstellen, der kein Analogon im Darm hat. Dieser mageneigene Polyp wird als hyperplasiogen bezeichnet, da er hyperplastische wie auch adenomartige Strukturen aufweist. Eine maligne Entartung desselben ist unwahrscheinlich. Der bösartige Polyp des Magens ist ein Frühkarzinom I, das sich wahrscheinlich über eine Drüsenhalsproliferation mit Zellatypie im Sinne der „borderline lesion protruded type„ entwickelt. Mit dem häufigen Nachweis multipler hyperplasiogener Polypen muß das Pauschalurteil: Polypose = Präkanzerose revidiert werden.
