Summary

A 44-year-old man presented with epigastric pains of one month's duration. X-ray examination and gastroscopy suggested an early gastric cancer type IIc in the upper part of the stomach, but guided biopsy and cytology showed no malignant signs. Histological examination of the resected stomach revealed a lymphosarcoma limited to the mucosa and submucosa. Early stages of lymphosarcoma and other malignant lymphomas present the same macroscopical aspect as early gastric cancer. Differentiation is possible, if at all, only by histological or cytological examination. Frequency, prognosis and the importance of correct preoperative diagnosis are discussed.

Zusammenfassung

Bei einem 44jährigen Mann, der seit einem Monat unter epigastrischen Schmerzen litt, ergaben Magenröntgenuntersuchung und Gastroskopie den Verdacht auf ein Frühkarzinom Typ IIc im oberen Teil des Magens. Gezielte Biopsie und Zytologie brachten keinen Hinweis für Malignität. Die histologische Untersuchung des resezierten Magens zeigte ein Lymphosarkom, das auf Mukosa und Submukosa beschränkt war. Frühstadien des Lymphosarkoms oder anderer maligner Lymphome, in denen die malignen Veränderungen noch auf Mukosa und Submukosa beschränkt sind, lassen sich makroskopisch nicht von einem Frühkarzinom unterscheiden. Eine Differenzierung ist nur durch histologische oder zytologische Untersuchung möglich. Häufigkeit, Prognose und die Wichtigkeit einer korrekten präoperativen Diagnose werden nach der Literatur diskutiert.
