Kolon-Triplikatur assoziiert mit einer Ileum-Atresie bei Laparoschisis

Zusammenfassung

In der vorliegenden Arbeit wird der außergewöhnliche und seltene Fall einer Kolon-Triplikatur mit assoziierter, vorher nicht berichteter Fehlbildungskombination einschließlich des klinisch-diagnostischen und therapeutischen Managements unter Berücksichtigung der einschlägigen, aber spärlichen medizinisch-wissenschaftlichen Referenzen beschrieben. Im Rahmen einer Ileum-Atresie bei Laparoschisis wurde pathohistologisch überraschend eine Kolon-Triplikatur festgestellt. Das chirurgische Vorgehen bestand nach primärer geplanter Sectio caesarea in der 34. SSW in der darmsparenden Resektion der Triplikatur einschließlich der Ileum-Atresie und Anlage einer Santulli-Anastomose mit späterem Ileostomaverschluss. Es handelt sich um den 9. berichteten Fall der zugänglichen englischsprachigen Medizinliteratur, einmalig bisher jedoch durch die Kombination mit Ileum-Atresie und Laparoschisis, der operativ erfolgreich zu versorgen war.

Abstract

The present paper reports the unusual and rare case of a colon triplication associated with other findings never reported before, including the diagnostic and therapeutic management with consideration of the relevant, but rare scientific references in the medical literature. In the basic finding of an ileum atresia with laparoschisis, a colon triplication was incidentally found. The surgical procedure performed after the primarily planned Caesarean section within the 34^th week of gravidity consisted of a bowel-preserving resection of the triplication including the ileum atresia and a subsequent Santulli anastomosis followed by a later excision of the ileostoma. Interestingly, this is the 9^th reported case in the accessible English medical literature but, however, the first and only one with an associated ileum atresia and laparoschisis, which was successfully treated with a surgical intervention.
