Postoperative Hämodynamik nach Mitralklappenersatz

Zusammenfassung

1. Laufende Registrierung der hämodynamischen Parameter, insbesondere von HZV und rechts- sowie linksatrialen Drucken, nach Mitralklappenersatz erlaubt eine optimale Ausnutzung der myokardialen Reserven in der postoperativen Phase.

2. Eine induzierte Hypervolämie bei übernormalen atrialen Drucken bedingt eine unmittelbare und anhaltende Steigerung des HZV in der post-operativen Phase.

3. Disparates Verhalten der atrialen Drucke kann dazu beitragen, myokardiales Versagen der einen oder anderen Kammer sowie Malfunktionen der Mitralklappenprothese bzw. der Trikuspidalis aufzudecken.

4. Eine systematische Messung der links- und rechtsatrialen Drucke sowie des HZV in der post-operativen Frühphase können als wertvolle Hilfe für die hämodynamische Führung der Patienten empfohlen werden.

Summary

1. Continuous registration of the haemodynamic parameters particularly the cardiac minute output and right as well as left atrial pressures allows an optimal exploitation of the myocardial reserves in the postoperative phase.

2. An induced hypervolaemia in hypernormal atrial pressures causes an immediate and lasting increase in the cardiac minute output in the post-operative phase.

3. Unequal behaviour in the atrial pressures can lead to detection of myocardial failure of one or other of the chambers as well as of malfunction of the mitral or tricuspid valve prostheses.

4. Systematic measurement of the left and right atrial pressures as well as of the cardiac minute output in the early postoperative phase is recommended as a valuable haemodynamic guide for the patient.
