Über die Ursachen des Laktatanstiegs bei extrakorporaler Zirkulation

Zusammenfassung

Der Laktatanstieg während der extrakorporalen Zirkulation in Form der Hämodilutionsperfusion ist nach Ausschluß eines unzureichenden Stromzeitvolumens und einer Hypoxie im wesentlichen durch eine Erhöhung der Blutglukose bei gleichzeitiger Hemmung der Pyruvatoxydation infolge bevorzugter Oxydation der freien Fettsäuren bedingt. Ein anaerober Laktatanstieg trotz überkritischer Sauerstoffdrucke ist zwar unwahrscheinlich, könnte jedoch durch eine lokale Blutverteilungsstörung zustande kommen.

If insufficient flow-time volume and hypoxia are excluded, increase in lactate during extracorporal circulation in with haemodilution perfusion is mainly due to an increase in blood glucose with simultaneous inhibition of pyruvate oxydation as a result of preferential Oxydation of free fatty acids. An anaerobic increase in lactate in spite of over-critical oxygen pressure is not probable but could occur through a local disturbance in blood distribution.
