Zusammenfassung
Einzelne Fälle von akutem Pyopneumothorax sowohl beim Säugling als auch bei Erwachsenen sind durch alleinige Drainage nicht besserungsfähig. Frühzeitige Thorakotomie mit Abschrubben der Fibrinklumpen insbesondere auch auf der kollabierten Lunge mit Hilfe von in Fibrolan oder Alphachymotrypsin getränkten Tupfern macht die Lunge wieder entfaltbar. Die kortikalen Abszesse werden dabei nur kürettiert, sonst aber belassen. Es wird über 10 Fälle berichtet bei einem Todesfall*).

Summary
Some cases of acute pyopneumothorax both in infants and adults cannot be improved by drainage alone. Early thoracotomy with disintegration of the fibrin clots particularly on the collapsed lung using swabs soaked in fibrolan or alpha-chemotrypsin renders the lung re-inflatable. Cortical abscesses are only curetted and otherwise left alone. Ten cases are reported including 1 death*.

Résumé
Quelques cas de pyopneumothorax aigüs du nourrisson et de l'adulte ne peuvent être améliorés par un simple drainage. La thoracotomie précoce avec détachement des dépôts de fibrine en particulier sur le poumon collabé à l'aide de tampons imbibés de Fibrolan ou d'Alphachymotrypsine permet au poumon de reprendre son expansion.
Les abcès corticaux ne nécessitent qu'un curetage et aucune autre intervention. Présentation de dix cas, dont in mortel, entre temps deux autres malades ont été traités avec succès par cette méthode.
