Die Beurteilung des „Rückfluß“-Testes nach akuter Okklusion eines Hauptkoronarastes

Zusammenfassung

Nach dem Mautz-Greggschen Test durchgeführte Experimente an Hundeherzen zeigt der Blutrückfluß zwischen dem Ramus descendens anterior und dem Ramus circumflexus der linken Koronararterie eine auffallende Abweichung, wenn die Werte auf 100 g Herzgewicht und Gefäßquerschnitt bezogen werden.

Wegen der Identität dieser Ergebnisse mit denen des Ligaturtestes kommt das Verfahren der Auswertung der Myokardrevaskularisationsoperationen nur als Zusatztest in Betracht. Es zeigt die Gefahrenzonen der einzelnen Myokardgebiete nur mittelbar an, weil der gemessene Rückfluß nicht auf die kapillären, sondern auf die größeren interkoronaren Anastomosen hinweist.

Summary

The Matz-Gregg Test was performed in dogs and a conspicuous difference in blood flow in the anterior descending and in the circumflex branch of the left coronary artery was found when the absolute values were related to 100 g heart wieght and diameter of the respective vessel.

These results are identical with those of the ligature test and thus this procedure is of value only as an auxiliary test in the evaluation of revascularizing procedures. It points out the danger regions of individual areas of the myocardium only indirectly because the flow measured does not indicate the capillary return but rather that in the larger intercoronary anastomoses.

Résumé

Nous avons expérimenté les tests de Mautz-Gregg sur les coeurs de chiens. Le reflux sanguin entre la branche antérieure et la branche circonflexe de l'artère coronaire gauche montre une différence frappante si on tient compte du poids cardiaque de 100 gr. et de l'incision des vaisseaux.

Les résultais des tests de ligature sont identiques. D'ou le procédé, en tant que test supplémentaire, de la mise en valeur de l'opération revascularisant le myocarde, vient seulement en ligne de compte Ceci montre que les zones dangereuses du coeur sont les zones moyennes. En effet le reflux sanguin mesuré ne se remarque pas au niveau des capillaires mais bien au niveau des grandes/anastomoses intercor onariennes.
