Moderne Serodiagnostik der Syphilis

Zusammenfassung

In einer Studie zur Lues-Serologie, an der sich zwölf Medizinaluntersuchungsämter und -stellen des Landes Nordrhein-Westfalen beteiligten, wurden der TPHA- und FTA-ABS-Test mit den sogenannten klassischen Syphilis-Reaktionen (VDRL-Test, Cardiolipin- und Pallida-Komplementbindungsreaktion sowie Meinicke-Klärungsreaktion II) verglichen. Dabei zeigten sich TPHA- und FTA-ABS-Test den konventionellen Methoden sowohl an Spezifität als auch an Empfindlichkeit eindeutig überlegen. Das Konzept einer modernen Syphilis-Serologie kann sich somit auf den TPHA-Test als Syphilis-Ausschlußuntersuchung, den FTA-ABS-Test als Bestätigungsreaktion und den VDRL-Test als Parameter für die Beurteilung der Behandlungsbedürftigkeit bzw. Wirksamkeit einer antisyphilitischen Therapie beschränken. Mit diesen Reaktionen, die alle herkömmlichen Syphilis-Tests entbehrlich machen, ist eine rationelle, zuverlässige und kostengünstige Serodiagnose der Syphilis möglich.

Summary

Twelve laboratories in the north Rhine-Westphalia participated in comparing TPHA and FTA-ABS tests with the so-called classical syphilis reactions (VDRL test, cardiolipin and pallida complement-fixation reaction, as well as Meinicke clearing reaction II). TPHA and FTA-ABS tests proved to be superior to the conventional methods both as regards specificity and sensitivity. Present-day serological tests of syphilis can thus be limited to the TPHA test to exclude syphilis, the FTA-ABS test as a confirmatory reaction and the VDRL test to judge the need or effectiveness of antisyphilitic treatment. These reactions which supersede the conventional syphilis tests serve for rational, reliable and cheap serological diagnosis of syphilis.
